White screen error when adding a custom field to a form question

Hello,

I am getting an error / fault when trying to connect a custom field to a form question (seems to be only fields I’ve created - ones that have worked for months on other forms).

The screen for the form creation goes white and I have to close the form and start again.

Currently I cannot create forms that link custom fields I have created.
