Briefly describe (1-2 sentences) the Bug you’re experiencing: some images are imbedded with the task name and some are attached into the task but I’m not sure of how it’s doing this because I’m using the same methods to attach.

Just to confirm, are you currently accessing your project in Board or List view? In Board view, you will only see one image per card and you’d need to access the task to see the rest of the images. It’d be great if you could send us a screenshot showing the issue so I can better assist you
