When a co-worker attempted to fill out a form I created, the answers would not stay in the response field. Sometimes answer ended up above the question. Also - even though the user said he responded to all questions - some had no answers at all. Plus I have no visibility to the questions in the form after its been filled out.
Can someone from Asana fill out my actual form for a first-hand experience?

Hi @Julie_Wanstedt, welcome to the Asana Community Forum 
From the screenshot you have attached, it looks like your colleague has edited the form instead of responding. The Forms page of the project is used to create/edit a form only. The form is not submitted via this page.
To submit a form, you can provide the link to the form to your colleague by using the Copy link button in the top right. Once they fill in the answers there, a task will be created in this project with their response.
