Our team of 8 persons are using asana and most have tried duplicating tasks by a mistake. Without them knowing that they duplicated it. It is a big problem for us that duplicated task occur again and again. So we have to delete them all the time. Nobody knows how it happens and is seems like most teammembers have tried it. We use tablets and phones (app).
Have anybody come across this issue?

Is it possible you have tasks with recurring Due dates and when you mark them complete, they duplicate with the next Due date recurrence? That’s actually expected behavior, but maybe your team is unaware of that and should not be using recurring tasks?
Also, a recurring parent task with recurring subtasks is problematic and should not be used.
Is it possible that one of these is what you’re running into?
