Attachments and comments are disappearing without a trace in random tasks.

I have encountered an issue twice now where attachments have completely vanished from a task, making it look as if I never uploaded them! A few comments have disappeared as well.

I have already checked deleted items, but they are not there. There is nothing in the project/task history showing any changes or deletions.

This is a big problem, as it is making it appear as if I’m not completing my work.
