Looking to use Forms to collect feedback - is there a way to trigger a form to be generated and sent to the person assigned to the task? I have a standard set of questions I’d like to ask the assignee about the project.

Hi @Jon_Allain welcome to the forum
You can’t ‘generate’ a form but if you set one up then you can copy the link of the form (it’s complete URL, top right of the form’s editor window) and paste it into a comment of a rule’s action.
i.e set up a rule that when triggered (from a custom field option or when you assign a task to a person or whatever suitable) then action is add a comment (available from the rules actions list) in which you can write a message eg. “Please fill out this form: [paste the form’s URL]”

When a form is submitted, it can create a new task. Is there a way to tie it back to the original task. I.E. if I have a task called “Document A” and it’s marked as complete, then the person fills out the form, can I link that to the original task “Document A” in some way?
I can’t think how that would be possible to achieve automatically with rules but you could manually move the task generated by the form as a subtask to the original task or use the ‘convert to… subtask’ from the task actions menu.
Also, from the form’s settings make sure you choose to name the task based on one of the form’s question, such as the submitter’s name for easy reference
