Yes, this happened to us too, someone deleted the form; thankfully I had a backup but the URL changes so I had to re-launch that to everyone, which isn’t ideal.

This feature is actually available on Asana Enterprise plans - the Customize menu can be restricted to just Project Admins, which disables any Editors from editing any features in the Customize menu, including Forms.
