Form's feature change?

Hello Everyone,
I dont know whats wrong with the form’s UI, I can see the interface like this.

I cannot see any textbox or field for date, normal text fields or more features like before. Is anybody else facing the same issue?

Yes, the UI has changed, or is undergoing an A/B test.

Click the “+ New question” and you will see a new way to add your question and it will offer the option to connect the question to a custom field or choose the different answer types.
