Form in project suddenly missing. Can't find a way to retrieve it.

We had a form in one of our projects that just suddenly disappeared and I couldn’t find a way to retrieve it.

I have no clue what happened. The form was being used to intake design briefs that’s being fed to the project. It’s very important that we retrieve the form as it was made with a bit of complexity which also affects the project dashboard data.
