I’ve been searching the help pages and it looks like questions & headers in Forms should allow for links to be created via the Rich Text Formatting, but I cannot seem to get this to work at all. Is there anything I’m missing / additional documentation available?

Yes, use the link icon in the toolbar to create a vanity link; you can start by selecting the text to add a URL to:
