Uploading files in a form

I am trying to create a from that acts as like a ticket request. I am able to capture all the information I need other than uploading a document. Does anyone know if its possible to add a document upload feature to the form so that people can fill out the Asana from with all the document needed to raise a ticket?

Welcome to the forum, @Becky_Mullings! Yes, there is an “Attachment” question type that will allow for document uploads.

Attachments are available to add to forms on all paid Asana plans (which is where forms generally are available).

However, your Asana (Super) Admin might have turned them off, so check with them:
