I wanted to show how we use a single form to provide multiple forms via a single link.
I just created a Form with multiple Selection, but then added no options to that.
By that you can create sth. that looks like this:
I had to pixelate the links but you get the point.
In editing mode it looks like this:

We use it so that we only have to give out one QR Code to our colleagues and not 5 different ones.
