I would like to know whether adding a guest, that is, someone from outside an organisation to an Asana project counts as an internal licence when you add them to the project?

No, they don’t, assuming your Asana is an “organization” (not a workspace), meaning that it’s based on your domain name (mycompany.com); if you see Teams under People in the left sidebar, you’re an “organization.”
If so, you’re only billed for “members,” those who log in to your Asana with an email address with your org’s domain name. All others (gmail.com, any other domain) are free and appear as “Guests.”
